Job Description

Day/Evening/Night/Weekend Varied Hours Home Health

Assists with personal care services.

Does light housekeeping, laundry and errands.

Assists clients in relearning ADL’s.

Assists with non-skilled procedures.

Prepares meals, assists with food and fluid intake.

Assists client in moving about and with prescribed therapy exercises.

Takes vital signs, records on proper form and reports unusual findings to RN.

Attends required in-services and case conferences and other meetings as directed.

Performs care and contributes vital information about clients to other disciplines to improve outcomes.

Cooperates with supervisor in planning weekly schedule, completes schedule in a timely manner, keeps appropriate patients and staff informed of changes.

Schedules visits with patient and/or family and arrives promptly at arranged time. Calls patient if an unavoidable delay of more than 30 minutes occurs.

Communicates in a clear and courteous manner with patients, families and staff members.

Communicates and documents patient care and findings in an accurate and timely manner, which includes documentation within 24 hours of service.

Maintains privacy and confidentiality of agency and client information.

Job Requirements

CERTIFICATES, LICENSES, REGISTRATIONS: Must have current Kansas CNA or Home Health Aide license. CNA’s must obtain their Home Health Aide licenses within 90 days of hire.

EDUCATION and/or EXPERIENCE: High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.

LANGUAGE SKILLS: Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one to clients and other employees of the organization.

COMPUTER SKILLS: Ability to operate computers to complete required paperwork and other relevant work.

REASONING ABILITY: Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations.

OTHER SKILLS AND ABILITIES: The Home Health Aide must be able to take a temperature, blood pressure, pulse and respiration and report and changes in a client’s condition to the registered nurse. The Home Health Aide shall follow the written plan of care that is written by the registered nurse.

While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel objects, tools, or controls; reach with hands and arms; and talk or hear. The employee frequently is required to stand and walk. The employee is occasionally required to sit, climb or balance; stoop, kneel, crouch, or crawl; and taste or smell.

The employee must regularly lift and/or move up to 25 pounds, frequently lift and/or move up to 50 pounds, and occasionally lift and/or move up to 65 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

TRANSPORTATION : The employee must drive to patients’ homes to deliver care. Dependable transportation, active auto insurance, and a valid drivers’ license is required.
