Provide personal healthcare and related services to the patient in their place of residence; to assist in providing a safe and clean environment, work cooperatively with patient and family and share observations and problems with the coordinator. Works under the direction and supervision of a registered nurse.
Job Functions –
Provides direct patient care to patient under direction of the RN and according to the Aide Plan of care.
Assists the patient with safe transfers and ambulation, per agency policy.
Appropriately reports changes and documents pertinent information about care rendered to patient to ensure continuity of care.
Practices accepted infection control principles. Provides a clean, safe and comfortable environment.
Promotes positive, supportive, respectful communication to patient/family and other employees.
Provides an environment which promotes respect for patient, privacy, and property.
Assists the patient with proper nutrition and adequate fluid intake.
Contributes to the management and efficient operation of the agency and demonstrates behavior consistent with HCM Values.
Assure continuity of quality patient care delivered with appropriate and timely documentation.
Collaborates with the IDT to effectively implement agreed upon plan of care and recommend revisions as necessary.
Education and Skills-
- Must have valid current driver’s license and proof of insurance.
- Must be able to read and write in English and follow written and verbal instructions in English effectively. Demonstrates interest in the welfare of ill and elderly. Must pass Aide Competency Evaluation Skills checklist that meets 418.76© and written skills test.
- At least one (1) year experience, preferred, as a Hospice aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long term care facility or completion of Certified Nursing Assistant or Certified Home Health Aide program.
- High school diploma.
- Successfully completed one of the following:
Training program/ competency evaluation as specified in Medicare regulations;
Competency evaluation program that meets 418.76
Nurse aide training /competency evaluation program approved by the state and listed in good standing on the state nurse aide registry;
State Licensure program that meets requirements of 418.76(b)(c)
